Your provider has offered the opportunity to obtain a score which assesses your risk of common disease based on your genetics.

Please enter your contact information and Provider ID (given to you by your UVA physician), and we will mail you a kit. Once you return the kit with a saliva sample, we will process the information and provide results to your provider in 4-5 weeks. Your provider will be able to review this information with you at your next scheduled visit.

Please note that we charge $200 for this testing service, which is a discounted rate offered exclusively to UVA Health patients. We will send you a link to complete payment for the test via email.
